Forms overview

This article will give you an overview of how forms work on GPsurgery.net website. It will also explain which forms are available to you on the standard plan, and how you can add custom forms to your website.


Our standard service includes a set of forms which have been carefully checked by our Clinical Safety Officer. These forms are usually added to your site by our support team. If you would like to add or remove a standard form, drop us an email at support@gpsurgery.net.


Our recommendations


We generally recommend that your form notifications are set up to send an email with the content of the form, and a PDF with the same information, to your shared practice email address/workflow every time someone completes the form. Our support team is happy to set this up for you. Drop us an email at support@gpsurgery.net for help with form notifications.


By default, the Family and Friends test results are kept for 60 days before they are deleted automatically by our system. All other standard forms are set to delete after seven days.


Custom forms


If your organisation requires forms other than our standard forms, it is possible to upgrade your subscription. Ask us about custom forms at support@gpsurgery.net.
